A City network will provide services to 600 at-risk or homeless persons who have experienced trauma,mental illness and/or substance addiction.

TRANSFORMATION has come to St. Louis…

Transformation is a new network of service programs in the City of St.  Louis, which provide services to assist adult persons who are homeless or at risk for homelessness who have experienced trauma, are living with a serious mental illness and/or substance addiction achieve healthy and satisfying lives.

Free, individualized, and confidential support services for persons living/staying within the City of Saint Louis are:

*Counseling and psychotherapy
*Needs assessment including health screenings and care
*Psychiatry services
*Co-occurring substance abuse treatment
*Information & referral to community resources
*Employment and housing supports
*Treatment for trauma
*Priority is given to persons with no insurance
*Advocacy at the client and systems level is also a key element of this program

Community Partners are: Places for People, Queen of Peace Center, St. Patrick Center, former Community Alternatives, City of St. Louis Homeless Services Division, Missouri Institute of Mental Health, and NAMI St. Louis.  Training is provided by Community Connections and Dartmouth College.
