Annual Action Plan
Every fall, the City prepares an application that outlines specific programs to be supported in the upcoming year and the amount of funding necessary to achieve results.
Consolidated Annual Performance and Evaluation ReportThis report documents the many activities, initiatives, and services that were made possible by the federal funding programs listed above. It also serves as an overview of development activity and municipal services that collectively help implement the City's Consolidated Plan.

Section 504 Self-Evaluation Survey
CDA requires its CDBG and HOME subrecipients to complete a Section504 self‐evaluation survey and if needed a transition plan. Documents must be submitted to CDA and maintained in subrecipient's files for review during program monitoring.
